How To Order By Fax
If it is more convenient, you may fax your order to
us. Simply write down the following information on
a single sheet of paper and fax it to 1-630-375-5995.
INCLUDE:
- Your name, as it appears on your credit card.
- The type of credit card (we accept VISA,
MASTERCARD, DISCOVER and AMEX).
- The credit card number.
- The credit card expiration date.
- Your home address to ship to.
- Your home phone number.
- Quantity and prices for the items you are ordering
(be sure to include shipping).
- Please be sure to sign the bottom of the order.
How To Order By Mail
If you wish to order by U.S. Mail, simply include the
same information as indicated above in the "How To
Order By Fax" section. You may include a
check or money order if you prefer not to pay by credit
